Check /etc/mail/access, and make sure you've done a "make" in /etc/mail to
bring the hashed databases up to date.

Check your /etc/sendmail.cw or /etc/mail/sendmail.cw or
/etc/mail/local-host-names file, and make sure all your system's names are
listed, including "localhost".
Also, make sure both your unqualified host name and your fully qualified
domain name resolve properly on your system.

Check your system mail log (varies, but usually something like
/var/log/maillog).
