I have a Thinkpad and when running Ubuntu Feisty I could plug in a second monitor, restart xorg and the second monitor would come up with the desktop.
Now, xorg starts on the second head, but no window manager. Just a black screen and the default "X" mouse cursor. Anyone understand the process that starts the window manager on the second screen? That would help in debugging.
