- extsizes: autopagewidth.sty, extbook.cls, extletter.cls,
extproc.cls, extsizes.sty don't have a license statement; not much
activity in the Debian bug report [3].

Several files in extsizes are under the LPPL: extarticle.cls,
extreport.cls, size{14,17,20,8,9}.clo. Seems good enough to me
(speaking as the maintainer of TeX Live). Of course, if you (or anyone)
wants to contact the extsizes authors to get them to do it perfectly,
so much the better.
Speaking as a long-time GNU maintainer, license-question-answerer, etc.:
it has never been a do-or-die requirement that every file have a license
notice, although it is certainly the best practice. Although Savannah
tries hard to enforce that (at registration time), the rest of the world
doesn't.
