you can overrule (all paper) variables in the \paper block of your \score.

for spacing, you can fiddle with (from init/paperxx.ly)

        basicspace = 8.\pt;
        arithmetic_basicspace = 2.;
        arithmetic_multiplier = 6.\pt;

for changing page sizes (paper.ly)

        linewidth = \staffheight/20.0 * \linewidth20;
        textheight = \staffheight/20.0 * \textheight20;

(ugh).

i'm not sure, though, what a ly2dvi will do with these when 
papersize=a4 (paperxx.ly) is also set.
