Accidental Alignment

The accidental offset is not implemented when the interval between the
notes is a 4th.  This one feels is wrong because the accidental symbols
clash.  According to Stone: pp53 accidental offset occurs at an interval
of a 4th or less.

i.e. <dis ais>

I have looked at the source and am trying to work it out.
