And the third are two cases

  - notes of one voice printed on different staffs; sometime this has been
    done using the fact, that the middle C has one ledger line if printed
    with bass clef and one ledger line if printed with violin clef. So
    they used two staffs with the vertical distance of three (or two? :-) halfnote
    steps
  - notes of different staffs connected by one beam indicating e.g. which notes
    belong to the right hand and which notes belong to the left hand.
    
In the first sub-case the purpose was only to print music with only few ledger
lines.
