For win9x users "midinotate"
(http://www.notation.com/midinotatefree.htm) is among the best MIDI
applications I've ever met. The programme used to be free but with the
release of v. 2.0 has changed to shareware. Many commercial notation
programmes (for example Finale and Sibelius) have MIDI import
facilities.
