I figured this out one time. A few months back the default changed to A4,
and I went throught the same problems. If I remember right, you just have
to specify 'papersize=letter;' or something like that in the paper block.
All my lily files are at home, or I'd look it up for you.

I don't think this is in the documentation (at least not anywhere obvious),
but you can often find answers to these questions in the example files,
since they usually work correctly.
I guess this may not be too helpful. Hopefully someone else will give a
better answer than this. Oh well.
