The difference is whether or not you start with a new (empty) file.  The
latter (New Account Hierarchy) will allow you to add additional sets of
accounts to your existing data file (adding new hierarchies).  So, for
example, you could start with the basic checkbook and then use Add New
Hierarchy to add, say, the 'common' sets of accounts to get the full set
of expense accounts.  Or you could add the home-owner accounts..
