It could also be a society which (i) has inadequate resources (ii) manages its limited resources in an inefficient manner or (iii) has used its resources to offer highly subsidised education to create medical skills which have then migrated to greener pastures.
Of course, there are n number of other permutations and combinations and other factors involved.
