Hi Richard,

Unfortunately, I'm having trouble reproducing these problems, which
makes it difficult to debug.  Can you set up a page that's resulting
in the "File not found" error so that we can take a look?

Also, if anyone else is experiencing this error, please post here so
we can get more data points.
