It's not possible, you'll have to traverse the index no matter what. I advise counting either offline or on write if this is possible. With very large numbers, the 90% use case is that you want an approximation. There may be techniques for approximating (similar to Google Search or Gmail), but these techniques are often very domain specific. For instance, you may look at the first 20 results, realize that they correlate to 5 parent elements, know that there are 100 parent elements and estimate the results at ( 20 / 5 ) * 100 = 400 results.
