I'm having the same problem for several months now. After doing some tests with colleagues and family, I found out that it only occurred with one certain ISP (Belgacom in Belgium) ...
I also see that the custom domain gives problems while the appspot domain works, but it is all difficult to test. I'm still investigating this, but any pointers to tools to log or diagnose this would be helpful.
