This appears to be caused by a load balancer issue that has been unresolved 
for several months 
now:

 As it turns out, I had another application which has had logins broken for 
some time now, thankfully I'm the only one that ever logged into it.

Posting this in here so others won't waste time on trying to get resolution 
to this issue.
