New issue 1906 by [email protected]: Better error if onFailure is not
supplied to caja.initialize.

caja.initialize blindly calls onFailure if initialization fails, resulting
in an "undefined is not a function" uncaught error. In order to help out
new Caja users who might often see this error, we should explicitly check
if the failure callback was not provided and log an appropriate error
instead.
