there are various event registration forms in google docs that can be
embedded on web sites or they can be set up as links to the forms
themselves.The forms can also be emailed. You can edit the forms to
add categories of your choosing.The responses to the forms can be set
to be shown on an i-google page if you add google docs to the i-google
page as an app and those responses are also held in the spreadsheet
attached to the form.
