Aaron, Did you ever resolve this? I'm having the same difficultly.
@Julian, The documentation you link to for authentication for images seems to suggest that an auth token needs to be passed as an HTTP header when fetching the image... but if we're using the JS API, there's never an opportunity to set a special header. Is it really not possible to create a URL to the image that will work for the authenticated user?
