New issue 779 by [email protected]: De-duplicate listeners and
interceptors

These patches add de-duplication (by equals()/hashCode()) for all listener
and interceptor types. It's potentially a breaking change, except for the
first patch because ProvisionListener isn't yet released.
I tried to add the logic in the right place but I can easily move it if
there's somewhere better.
