I am not aware of this "well-known" issue. Here is an example that show custom tile at the beginning.
http://gmaps-utility-library-flash.googlecode.com/svn/trunk/examples/ArcGISLink/bin-release/MultiService3D.html I did not test every API and flash version, but I never had that problem except when API changed from 1.09 to 1.09a, some custom tiles not showing if the scale between layers are not 2. It was fixed immediately.
