It's quite possible that you have the right code, but because of your
web host you will never get it working.
Your HTML page, and the output from your PHP have this appended to the
end:

hosting_ads/gd01.js'></script>
This doesn't make too much difference on your HTML page, apart from
making it invalid code, but it completely invalidates the XML so it
will *never* work.
