If it's intermittent, then a probable cause is arriving at the map from a link in a Flash object. Apparently Flash doesn't allow the referer header to be set correctly, which causes the server-side key- check to fail. A solution to that is the kludge of linking instead to an interstitial HTML page which immediately redirects to the map page. Issue 1092 http://code.google.com/p/gmaps-api-issues/issues/detail?id=1092 indicates that the key-error messages may be revised in the next release, so this might be included.
