These two lines fail

  var inputProvince = document.getElementById('inputProvince');
  if (inputProvince.value == "ON"){

because at the time that code runs, the inputProvince input-box has
not been encountered, so getElementById returns null and there is
no .value property to test.

You need to put your code at the bottom of the page, just before </
body>, or run it in an onload function.
