You're sending every address to the geocoder to get the location,
every time anyone opens the page.

There is a limit to the number of geocode operations per day (2500)
and per second (varies, but around 30). You're falling foul of the
second, and probably falling foul of the first. Most of the geocoder
responses I got were "Over rate limt". In fact I'm rather surprised at
the number of markers which *are* shown.

Geocode your locations ONCE and store the locations in the database.
Then you can simply place a marker at the location you already know
about without bothering the geocoder at all. Result: all your markers
are shown, and far more quickly than waiting for their location would
allow, AND you don't use geocoder resources that others have a better
use for.
