The original example has no doctype. That is rendered as HTML 4
document. Changing the doctype to XHTML 1.1 means that the
"map_canvas" size definition must be adjusted accordingly.
Height is 100% of parent node height which is undefined now. With that
doctype you should add css style
html, body {height: 100%;}
