I am having a similar issue, I had narrowed it down to me switching the image or repositioning, but if its just the repositioning that helps me narrow down the issues.
I have a similar playback feature for showing a driving route and have recently moved from v2 to v3 of the API and have the same issue you are having. I only change the image if the heading is 15 degrees different than the previous heading and then reposition the image and I too am getting a constant "flicker" effect.
