I wanna made something like this:

http://dev.hillsborough.wateratlas.usf.edu/datamapper/

there will be: a select for the "code"; a select menu for the nation,
a select menu for the nation and a select menu for the app/subapp

there will be also a check box for the status of the markers.

I think that now i'm explained better.
