Aha! A friend found the problem. There were a few minor syntax bugs, but the
main issue was that I named some of the MySQL fields of my database as MySQL
keywords (I believe "where" and "when").

The solution was to put brackets around them in generating the php URL for
adding a row to the mysql table:

[when], [where]
