According to the Google TOS 10.5.d 
<https://developers.google.com/maps/terms> , one cannot store Content 
provided by the service. 

I cannot imagine another use for a form than collecting data from a user 
and storing it to a database. 

Is using the autocomplete address form 
<https://developers.google.com/maps/documentation/javascript/examples/places-autocomplete-addressform>
 
against Google TOS?
