Do you mean that the form only accept the date-format you choose? So that 
they only can fill 1/16 and not 1/16/12 or something? Its not possible I 
think. You have to tell them by the helptext, and exlain it there.

Or, use one checkbox for date (1-31) and one for year. Then sum this in 
spreadsheet??
