In htis xml file you also have to add references to the javascript ext
library :
<stylesheet src="js/resources/css/ext-all.css" />
<script src="js/ext-base.js" />
<script src="js/ext-all.js" />
depending where you unzipped the ext library containing these files.
There should be an other error, since I also have your problem
