I've done some more testing.
The problem only occurs when I use a PushButton with an image from an
ImageBundle,

public class AddButton extends PushButton{
        static Image image = ImageCollection.getImagesInstance().add
().createImage() ;

        public AddButton() {
                super(image) ;
        }

}

when I use a PushButton with a text

public class AddButton extends PushButton{
        static Image image = ImageCollection.getImagesInstance().add
().createImage() ;

        public AddButton() {
                super("add") ;
        }

}

setVisible(true) works as expected.

The setVisible is called from a changeListener on a ListBox. I've
moved the setVisible() into a deferedd command. The button is then
made visiblle, but the imag ein not displayed.

Any suggestions?

Thanks,

Danny
