In the example you cite, does UPLOAD_DIRECTORY (FileUploadServlet, lines 9 and 43) exist and does the user Tomcat is running as have write permissions to it? If not, correct that or use javax.servlet.context.tempdir (see http://commons.apache.org/proper/commons-fileupload/using.html).
