Until this form is filled out and we have reviewed your application,
you WILL NOT be listed in the /health services directory:
http://services.google.com/events/googhealthservices

Is there a reason why you can't use /h9 with a production
server?  Developers typically upload a cert to start testing
secure tokens on /h9.  You can request your domains be
registered for /h9 here:
http://services.google.com/events/googhealthdevelopers
