The CCR contains a <Comments> tag for comments which link to  
individual data elements (Allery, Medication, Problem, etc.).  / 
ContinuityOfCareRecord/Comments.  As for provider notes, the CCR is  
not intended to send full progress notes or History & Physicals, or  
etc.  One can place important notes that resulted from an encounter  
in /ContinuityOfCare/Body/Encounters/Encounter/Description
