@ Daisy, Do you mean an online form that you fill out that pushes the data from the form fields to a Google Health record ?
Or a PDF form that you might fill out and then submit to Google Health record ? I have access to PDF forms that can export to valid ASTM CCR data and HL7 CDA XML data... While Google Heath does not support HL7 CDA XML data directly, one can upload PDF files to a Google Health record, and a PDF XFA form can contain (import and export) valid HL7 CDA XML data files...
