To answer diagnosis questions for #144, I attempted to add a diagnosis CDM
view (see [https://bitbucket.org/njgraham/pcori-annotated-data-
dictionary/commits/eb6e04281dfd9fd92758da1d51c7baf1d38e8e2e eb6e042]) much
like I did for demographics and encounters. Unfortunately, querying this
view results in really bad performance - I let it run for several minutes
before killing the session. I plan to take a look at the execution plans.
Without index hints, some of the other queries in the same file took a
long time so maybe Oracle has generated an non-ideal plan and I can make
it better with some tuning.
