Please follow, the REDCap project setup process

  1.  The KUMC provided data 
dictionary<https://informatics.gpcnetwork.org/trac/Project/attachment/ticket/208/PCORIALSREDCapSurvey_DataDictionary_2015-03-23.csv>​<https://informatics.gpcnetwork.org/trac/Project/raw-attachment/ticket/208/PCORIALSREDCapSurvey_DataDictionary_2015-03-23.csv>
 will be used to create the survey project.
  2.  Then the "ALS Clinic Survey" instrument will be enabled as a survey.
     *   Survey title: Patients ALS Clinics Survey (PALS)
     *   Survey Instructions: Please read the consent below.
     *   Survey Acknowledgment Text: Thank you for your time. We appreciate 
your effort with this survey. Have a nice day!
     *   Question Numbering: Custom Numbered
     *   Allow survey respondents to view aggregate survey results after 
completing the survey? DISABLED
     *   Allow 'Save & Return Later' option for respondents? NO
  3.  The field "email" will be designated as the email field to use for 
invitations to survey participants.
  4.  Then participant identifier option will be enabled in "Manage Survey 
Participants"

Once the project has been set up, please send me the screen shots of the 
following pages WITHOUT ANY PATIENT INFORMATION (examples shown below)

-          Survey Settings

-          Project setup

-          Participant List (in Manage Survey Participants)

If your REDCap pages look different or if you have any other questions I would 
be glad to help you set it up (Please be ready to share your screen during the 
dev call).
