Marshfield likes the SCILHS approach, which includes 2 variables instead
of 3. Tobacco Type is calculated based on the values for Smoked Tobacco
Status and Non-smoked Tobacco Status.
We would want to make minor adjusts to the values to match PCORnet CDM v3.
