This apparently led to mismatches between the survey and registry/EMR files (so that one patient's data was incorrectly paired with another patient's data). As I mentioned, our primary effort right now is to carefully redo the survey/registry linkage with data that comes directly from our oncology registry. The secondary concern is finding out what happened, but we are working on that, too. Our approach is to track the linkage through the various back-and-forth steps that took place from the initial identification of eligible cases through the creation of the final datamart.
