 Bret Gardner and I tested several rounds of i2b2 query development against
 our database.  I reviewed clinical records to refine the accuracy.  Bret
 has posted the i2b2 query print on Babel and I enclose a zipped copy (web
 archive page format) with this ticket.  I have edited in the coded
 valuesets (ICD9CM, ICD10CM, ICD10PCS, RXNORM, SNOMED CT and LOINC) for all
 relevant criteria.

 This posted version of the i2b2 query yielded a false positive rate of
 2/25 (8%) that was due to the fact that we do not have med reconciliation
 data stored in i2b2 yet.  Those sites that have such data should be able
 to do better.    I also found 2/25 cases where I think the clinician was
 on shaky ground when they recorded that the patient had a heart attack,
 but I chose not to second guess the healthcare provider for this
 validation round.
