1. Denominator or study population
Minimum two encounters (inpatient, outpatient, and emergency ̶ see Part A1 in 
Appendix A) in EDW at any time on different days.
AND age between 18 and 89 years old at time of encounter. Age is defined as a 
difference between the date of birth and date of encounter 
Note: Change on age definition from Supreme-DM to current version is important 
in order to ensure exclusion of <18 year old.
2. Diabetes Mellitus (DM) definition
Age between 18 and 89 years old at time of encounter AND
Any two of the following on different days, which are less than or equal to two 
years from each other: 
Type 1 and type 2 DM diagnoses (Part 2 in Appendix A)
HbA1c ≥ 6.5% (Part 3 in Appendix A)
random glucose ≥ 200 mg/dL (Part 3 in Appendix A)
fasting glucose ≥ 126 mg/dL (Part 3 in Appendix A)
OR by itself: DM-specific medication (Part 4 in Appendix A)
OR not specific DM-related medication (Part 5 in Appendix A), plus one other 
indicator (DM diagnosis, HbA1c test, random glucose test, fasting glucose test, 
or OGTT on a different date
