Let us get our own geocoded data first, work with that, and then we can provide 
format details.
That's hard to do without doing this ourselves first.

 The docs from Van Riper seem pretty clear to me. The only part that's left  is 
the details of how you want the data formatted when we submit it. Would  you 
please sketch an example file?


 Van Riper sent documentation and a link to data Wednesday, January 25,
 2017 2:35 PM. The documentation is in the next-d "Geocoding and MPC"
 folder.

 The docs include an ER diagram and a pretty clear explanation:

 > The GPC group will need these Stata files and the new census crosswalk  to 
 > link the socioeconomic data in the Stata files with their patient  geocodes. 
 > The socioeconomic data is for a more recent version of the  American 
 > Community Survey than they currently have. Due to some changes in  
 > geographic codes introduced by the Census Bureau, their existing census

 The data is in stata format, which I have never dealt with, but I see
