I think the former but DRNOC could clarify.

If you were ok with letting sites use it without further review you'd just 
populate
PROVIDER_NPI
with the real NPI

PROVIDER_NPI_FLAG  would let you indicate you actually have it but there would 
need to be some negotiation or added review to get it

Does anyone have a read on what the PROVIDER_NPI_FLAG  is supposed to mean?  
Are they talking about whether we have data or another site can use the info?
