New description:

encounters missing information on insurance used. Clearly, PCORI table
ENCOUNTER could not be used for next-D project to report insurance.
We ask sites to respond to this ticket with timelines they might need in
order to remap source insurance values to PCORI categories.
For Next-D purposes we would ask to remap into following categories:
