Hello everyone, We will be reimbursing all patients who attended the Learning Engagement Conference last week from GPC central, so I will be handling that process. In order for patients to be reimbursed, I will need a signed W9 form for each of them, along with original receipts (except for meal receipts - I won't need these as we will reimburse per diem). The W9 form can be scanned and emailed back to me, but you must mail in original receipts. The address to mail these to is:
