Gnumed uses a standard database format which can access databases over any network. (including the internet) In fact the demo program uses a remote DB site in Germany. The record can even be distributed to various DBs (though I think this is not industrial strength yet). Many Drs are creating the GPMP as a standalone document outside the medical records system - could we create a tool to make a GPMP to give to the patient with suitable format and a web export facility?

Using an agreed template a "Pitifil on steroids" could extract the data
and lob it to the central server.
The million-dollar question is: who gets access to the server, and
how?

would you give a PIN number to the patient, to give to the specialist/admitting
registrar?
or would you let the appropriate people (say ED charge nurse) have a password allowing access to all summaries from GPs in the area? With an automatic mailback to the GP whenever a file is accessed by a hospital person as a security measure.
