Gnumed uses a standard database format which can access databases over
any network. (including the internet) In fact the demo program uses a
remote DB site in Germany. The record can even be distributed to various
DBs (though I think this is not industrial strength yet).
Many Drs are creating the GPMP as a standalone document outside the
medical records system - could we create a tool to make a GPMP to give
to the patient with suitable format and a web export facility?

If there are functional web accessible records systems - like
v-practice - which I must confess I have not got into serious looking
at. ( currently working ok, i understand)
If there are processes that could transfer an patients information
from a MD xml export, or a BestPractice xml export or whatever records
system - into the web accessible records system.
If part of getting the money for an item 723 included putting that
patients data into the web accessible record.
If HIC paid to allow any appropriate institution access to those records.
Then we would have the problem solved for our complex patients.

Sure I accept that the majority may not - I am referring to the
complex patient seeing multiple providers. These patients seem more
willing to accept the need for such a system (at least the ones I
have spoken to) - obviously proper security and consent procedures
would have to be followed. There seems to have such slow progress on
a networked record - I think a interested group of amateurs could
get something going when the huge investment has achieved little.
When I was in Broome recently I spoke to a rural doc who worked in
the Geraldton region. They had shared on call over an area of several
hundred km. He said they had set up a networked system with a patient
summary several years ago - it seems it wasnt that hard. Dont know
what happened to it tho..

Ian has asked the BIG question.
Why would patients who wished to keep their clinical details
confidential want a GP to post THEIR medical details anywhere in
cyberspace?

These GP Management Plans seem tailor made to be posted on the net
as a
basis for a distributed health record a la HealthConnect. Add a
medication list and you have a useful resource for patient management.
What would it take to do a demo project to make them available on the
net using servers hosted by list members?
Using an agreed template a "Pitifil on steroids" could extract the data
and lob it to the central server.
The million-dollar question is: who gets access to the server, and
how?
would you give a PIN number to the patient, to give to the
specialist/admitting
registrar?
or would you let the appropriate people (say ED charge nurse) have a
password
allowing access to all summaries from GPs in the area? With an
automatic mailback
to the GP whenever a file is accessed by a hospital person as a
security measure.
