VoIP doesn't just mean cheaper outward calls. In our case I hope it will
  eliminate the need for a second PABX at a branch practice.
Until Jan 2005 our practice provided infrastructure services to the
general practice at the local hospital. Rather than have a dedicated
PABX at the branch for what was always likely to be a short term
arrangement we used Telstra's Centel system (an exchange based PABX
arrangement for which you pay a premium on the line subscription) to
link to 3 standard phones at the branch.
The additional cost of this to our phone bill was about $500/month (line
rentals + calls),  not including the ADSL at the branch.

We are now about to establish a permanant branch in a town nearby but on
this occasion we're going to equip the branch with four IP handsets
which interface to the private side of the new PABX at the main surgery
via VPN. These cost about $600 per seat including the handsets and
licences, but the ongoing costs are negligible.
They have full functionality compared with the on-site standard digital
handsets .
I've had 3 of these IP sets on trial at home for the last month. The
quality is satisfactory even if I select an outward VoIP line for an STD
call - (meaning there are 2 VoIP hops involved, one from the IP handset
to the main PABX and another from the PABX to the target number).
The cost of this sort of technology has now dropped to the point where
it is an ideal option for multi-site practices who are upgrading their
PABXs. Ideal, that is if you don't want to roll your own asterix system.
