Shared EHR - US EHR Vendors roadmap and supporting documentation

 

 

The US based HIMSS Electronic Health Record Vendor Association (EHRVA:  http://www.himssehrva.org/ASP/index.asp )  this week released two documents which may be of interest to HL7 members and health informatics industry:

 

  1. The EHR Interoperability Roadmap

 

The vendor association represents 21 of the major US EHR vendors including Cerner, Eclipsys, Epic, IDX, McKesson, Siemens and GE. This document is their view of the interoperability framework for EHR. It is based on the Integrating Healthcare Enterprise (IHE) architecture which was showcased at the HIMSS 2006 interoperability demonstration.  This profile is being considered for inclusion in the HL7 Aust/HISA 2006 demonstration next August.  This model is based on an approach where clinical documents eg discharge summaries, pathology reports, radiology reports and images  are stored on the web servers of the originating healthcare agency (or a regional or central system if such evolves), indexed by a document registry (not dissimilar to the Web search engine approach) and accessed by participating health services.  It would support access initially to unstructured and non standardised document (in human readable formats eg word documents, pdf, images  and provide a pathway to adopting standardised approaches such as the CEN 13606 or HL7 CDA v2  as these develop to support both readability and computer data processing. This staged approach is seen by the EHRVA as a pragmatic and achievable in 2006 with a future pathway to closer integration of healthcare information.   1. Quick start Guide to HL7 Clinical Document Architecture.

 

HL7 CDA is a standard for XML representation of Healthcare documents. This guide was developed to assist vendors implement the HL7 CDA which is a core component of the IHE document sharing model. http://www.himssehrva.org/ASP/tools.asp#quickstart

 

 

 

Note:       The USA is moving to regional health information exchange based on the above type of approach, where as the UK is basing their EHR interoperation on a central EHR spine linking all NHS systems.   Australia has been grappling for several years with how to achieve a shared EHR through the activities of HealthConnect and various State based initiatives.  The National E-Health Transition Authority (NEHTA) have issued a paper developed by DH4 Consulting on options for an interoperability framework for EHR in Australia. http://www.nehta.gov.au/content/view/66/115/. HL7 News:
