I find it quite amazing/depressing how little progress there has been on
this issue in the time I have been in practice (28 years)
If anything the standard overall is possibly worse thsn it used to be.
Surprisingly Royal Perth Hospital is a standout performer - I think they
now have excellent discharge summaries (some of our patients come from
there )
Bourke was interesting - all the towns records on a hospital server! I
spose that is possible in a rural setting.

possibly SADI and perhaps the
most successful is Peninsula Health.
