I didn't suggest your service should be free. I was giving Tom a recipe for global domination of his market, you are welcome to use it as well.

It goes like this:

1. You provide high quality messaging for GPs and Specialists at no cost. It doesn't even have to be open source. This will cost you some money but you no longer need to put resources into the hardest part (sales), so it saves you some too.

2. Your product grows exponentially until you have almost blanket market share.

3. A big fish decide they want to do something very similar on a big scale, let's say, the government and electronic discharge summaries. They see your dominance in the market and guess who gets the tender with all the directions left in?

This is a time tested formula for success. The trick is getting to point 2 before your competitor gets as smart as you.
